 REGISTRATION INSTRUCTIONS:

To complete the REGISTRATION process, you must complete the WORKSHOP REGISTRATION FORM below, and provide tuition payment.    Tuition may be paid by credit card (VISA, MASTERCARD, DISCOVER, AMERICAN EXPRESS),  PayPal, or you may pay by check.     The total tuition cost is $95.00 per person.  You may elect to pay the full tuition now, or you may pay 50% of the tuition ($47.50) now, with the balance due the day of the workshop. 

CANCELLATION POLICY:  You must notify us via email ( postmaster@ECGTraining.org ) or phone (1-888-727-6910) no less than 3 days (72 hours) prior to the scheduled start time of the workshop in order to receive a full refund.  For cancellation notifications made less than 72 hours from the scheduled workshop, a partial refund will be made.  For "NO SHOWS," there will be NO REFUND.

NOTE:   When you sign up for a workshop, the basic registration fee includes one copy of our "12 Lead ECG in Acute Coronary Syndrome Workbook."  This 156 page color book is all you need to take this course.     At each workshop, we will be selling new hardcover editions of our textbook,  "12 Lead ECG Interpretation in Acute Coronary Syndrome with Case Studies from the Cardiac Catherization Lab."   The cost to purchase this book at one of our workshops is $55.00.    If you wish to purchase this textbook without attending a workshop, you may do so by clicking on the TEXTBOOKS link located along the left side of the screen.   Please note that this book's retail value is $123.00, and our regular website special price is $59.95.  On this website, we also sell the paperback version for $49.95.
